The Community of Colours Family Picnic in the Park featured an impactful Health Outreach Program, organised to promote health awareness, providing basic health checks, and empowering individuals to make informed decisions for healthier lifestyles.

 

Program Objectives

The health outreach initiative was designed to:

  • Engage the community through accessible health education.
  • Raise awareness about risk factors for common chronic diseases.
  • Encourage preventive health behaviors and lifestyle changes to reduce the risk of long-term illnesses.

The program provided attendees with opportunities for essential health checks and personalized medical advice, helping them take proactive steps toward improving their overall well-being.

Health Checks Offered

The following health screenings were conducted as part of the program:

  • Blood Pressure (BP) Check: To screen for hypertension and promote cardiovascular health.
  • Random Blood Glucose Check: To identify potential diabetes risks.
  • Lipid Profile (Cholesterol Check): To assess cholesterol levels and prevent cardiovascular disease (CVD).

A total of 14 community members participated in the health checks, with several key findings leading to personalized medical advice and signposting for further care.

  1. Elevated Blood Pressure

    • Four participants were found to have elevated BP, placing them at increased risk of hypertension.
    • Each individual was provided with advice on lifestyle changes, including dietary modifications and physical activity, and referred to their GP for further evaluation and management.
  2. Abnormal Blood Sugar Levels

    • Two attendees had abnormal blood sugar levels, indicating early-onset diabetes (pre-diabetic stage).
    • They were educated on the importance of managing blood sugar through diet and exercise and referred to their GP for follow-up care.
  3. Elevated Cholesterol

    • One participant exhibited elevated cholesterol levels.
    • They were advised on the risks associated with high cholesterol and educated about reducing saturated fats, choosing healthier meal options, and scheduling a recheck in six months. A QRISK assessment with their GP was also recommended.

Conclusion

The health outreach program at the Community of Colours Family Fun Picnic successfully engaged attendees, educated them on health risks, and empowered them to make positive lifestyle choices. This initiative highlighted the importance of early screening and preventive care in reducing the burden of chronic diseases.

With the encouraging outcomes of this program, future health outreach efforts are being planned to expand screenings and ensure more community members can benefit from these vital services.
